Download a Gifting Form below, get the necessary information for sections 1-3 from the Account Holder, and mail the completed slip along with your gift contribution to the address indicated on the slip to contribute to an existing Maryland College Investment Plan or Maryland Prepaid College Trust Account.

As we prepare for the transition of program management services to Intuition College Savings Solutions, LLC, we are no longer accepting Account Services request forms for the Prepaid College Trust.
Requests received in good order prior to 4:30pm on October 22, 2021 will be processed. If a request is received and is not in good order, attempts will be made to retrieve the missing information or documentation, but we cannot guarantee that the request will be processed if it was not submitted in good order.
All outstanding and/or not-in-good-order requests will be transferred to the new Program Manager for review beginning November 9, 2021.
